^Part of this is because few non-Europeans make it to the podium, and consequently don't get tested.

Kazakhstan is not in Europe, and Georgia is generally not considered (geographically) part of Europe either. However - sports federations often include Turkey, Georgia, Azerbaijan, and Armenia in their "European" championships - likewise, for political reasons, Israel.

It's tough to catch North Americans when they can't mane it past the preliminary rounds.

28 eastern Europeans tested negative and finished higher than North Americans. It does seem that they have a higher tendency towards PED's. But make no mistake, the best natural armwrrstkers come from Eastern Europe. Many North Americans are on PED's and still won't crack top 3 at worlds.
